Comfort Sisi Healthcare Inc (CSHI) was founded in Hyattsville, Maryland, by a group of individuals who have committed themselves to take care of people with disabilities and the seniors promoting their well-being. For years, Comfort Yeboah worked as a Medication Technician at several healthcare organizations. The name “Comfort Sisi” reflects the desire and willingness to build a company whose employees would be willing to promote the comfort of clients serving them compassionately. CSHI‘s mission is to function as a vital member of the DDS/DDA partnership with people with developmental disabilities to help participants lead personally defined and fulfilling lives by ensuring the highest quality of program service delivery on-going, consistent basis to each participant and family. 

CSHI‘s business is centered on collaborations with local, state, and federal agencies whose operations focus on people with disabilities. It aims to equip its employees with the needed knowledge and skills to provide high-quality person-centered professional, reliable, and innovative services and supports to individuals in tandem with the requirements or regulations of the state. Our goal is to provide choices and protection to individuals and ensure that they receive services in the most integrated setting appropriate. CSHI seeks to maximize participants’ ability to make choices while ensuring that those choices do not endanger the participants or others. We work closely with participants and state agencies willing to adapt our services to respond to each engagement’s shifting priorities.

In DC

Individualized Day Supports
CSHI offers these services to increase residents and community integration and provide individuals the ability to integrate fully into the community they find themselves to enable to contribute meaningfully to their families and communities and develop a full life within the community.

In-Home Supports
CSHI provides aides to assist people in their own homes or live in a family member’s home by giving personal care and supporting individuals with their activities of daily living (ADL), helping them achieve their goals, and improving adaptive skills, and participate in community life and activities. This service includes those that help promote people’s safety, independence, and community integration as established in their support plan (ISP). These services are designed to assist people with their activities of daily living such as bathing, transferring, dressing, grooming, and assisting with meals and the difficulties with incontinence.

Residential Habilitation
CSHI supports 4 to 6 persons in its residential facilities, offering habilitation services, personal care, nursing, and other residential supports, including but not limited to providing transportation. We offer these services in our residential facilities. The services include those that promote residents’ safety, independence, community integration as established in their support plan (ISP). CSHI supports individuals with their activities of daily living (ADL) to help them achieve their goals, improve adaptive skills, and participate in community life and activities. These services are designed to assist people with their activities of daily living such as bathing, transferring, dressing, grooming, and assisting with meals and the difficulties with incontinence. They are also meant to help participants develop and maintain community membership skills through engagement in community-based activities and programs with people without disabilities. Through these services, participants are provided with the opportunity to acquire the development of skills and social supports needed to gain and maintain a job and improve their self-advocacy.

Supported Living Services
We will provide services for one to three people in our residential facilities and provide habilitation, personal care, nursing, and other residential supports, including but not limited to providing transportation.

Day Habilitation Services
We will provide our clients the ability to integrate fully into the community they find themselves to enable them to contribute meaningfully to their families and communities, and develop a full life within the community.

Companion Services 
We will promote residents’ safety, independence, and community integration as established in their support plan (ISP).

Host Home Services
We will provide a family-type setting to support their clients with their activities of daily living (ADL), to help them achieve their goals, improve adaptive skills, and participate in community life and activities.

Personal Care Services
These services will be crucial parts of residential and in-home services, which will be designed to assist people with their activities of daily living including bathing, transferring, dressing, grooming, and assisting with meals and the difficulties with incontinence.

In Maryland

Community Living Group Home Services
This is CHSI‘s signature program. Services are meant to help participants develop and maintain the skills needed to do activities of daily living, instrumental activities of daily living, and socialization (in community residential settings). These skills are developed based on participants’ individualized goals and outcomes in their person-centered plan. They are limited to learning socially acceptable and standard of good behavior, learning effective communication, learning self-direction, problem-solving skills, and engaging in safety practices. We also offer nursing support services using registered nurses (RN), RN care manager, and/or RN case manager delegating nurses. These services ensure that the outcomes of the goals of participants’ quality health are improved.

Supported Living Services
The supported living services are meant to promote participants’ independence, enhance their ability to perform daily activities of daily living, and engage independently in their communities while learning some skills to explore employment opportunities. Services are meant to help participants develop and maintain community membership skills through engagement in community-based activities and programs with people without disabilities. CSHI also provides participants with the opportunity to acquire the development of skills and social supports needed to gain and maintain a job and improve their self-advocacy skills.

Residential Services for Seniors and People with Disabilities
Residential services include habilitative, community, supported residential services, that will encapsulate companion services and personal care services. We will provide a home for 4 to 6 persons in our residential facilities and provide habilitation, personal care, nursing, and other residential supports including but not limited to providing transportation.

The community and rehabilitative residential services are provided to enable individuals to have the ability to integrate fully into the community to enable them to contribute meaningfully to their families, communities, and develop a full life within the community. We intend to offer services to increase residents, community integration as and when necessary.

These services include assisting people with companion services, household services, and personal care services.

Home-Support Services for Seniors and People with Disabilities
Home-support services include housing supports, host-home support, in-home support, supported living, and personal support services mostly for people with disabilities.

Supported living services
we will provide services for one to three people in our residential facilities and provide habilitation, personal care, nursing, and other residential supports, including but not limited to providing transportation.

Host home services
We will provide a family-type setting to support their clients with their activities of daily living (ADL), to help them achieve their goals, to improve adaptive skills, and participate in community life and activities.

In-home support services also shall include companion services, household services, and personal care services. We will promote individual safety, independence, community integration as established in their support plan (ISP).

  • Running errands (including prescription pick-up, dry cleaning, etc.
  • Organizing and reading mails
  • Grocery shopping
  • Laundry and ironing
  • Escorting to appointments and social events
  • Assisting with pets’ care
  • Planning, preparing, and cleaning up meals

Personal Care Services

  • Provision of companionship and conversation
  • Helping with medication reminders
  • Assisting and monitoring with diet and eating
  • Helping with entertainment needs, such as games, crafts, reading, movie attendance
  • Assisting with clothing selection
  • Arranging for medical, dental, or other professional appointments
  • Disabled care: care for accident victims, people recovering from surgery, and other forms of medical procedures, etc.
  • Cooking: preparing all manner of American dishes, as well as non-American ones.
  • Homemaking: includes dishwashing, pet care, housekeeping, laundry, dusting, answering telephone calls, etc.
  • Transportation: transporting clients to and from doctor’s appointments, grocery, shopping, etc.
  • Specialty cleaning: such as housekeeping and maintenance services.
  • Visiting exciting places: the services offered would include visiting fascinating and enjoyable places of pastime, flexible eating plans with an ever-changing menu of mouthwatering sections, and creative facilities
  • Assist with a safe bathing environment
  • Assist with daily grooming and hygiene needs
  • Assist with dressing
  • Posture positioning
  • Eating assistance
  • Toileting assistance

Future-Advanced and Professional Services
We plan to collaborate with other agencies in providing:
1.Professional services including:

  • Art therapies
  • Behavior supports
  • Dental
  • Family training
  • Parenting support
  • Wellness services
  • Speech, hearing, language

2.Future advanced nursing care, including but not limited to:

  • Wound care, Alzheimer’s and dementia care, medication management, ventilator management, and other medical and non-medical support.
  • Skilled nursing:
    • Tracheotomy and ventilator care
    • Administering medications and treatments
    • Tube feeding etc.
  • Other services:
    • Outpatient and inpatient rehabilitation to support physical, speech, and occupational needs
    • The rehabilitation care is meant to provide 24/7 medical and nursing care and supervision including therapy programs for patients after serious injury and illness with the focus on regaining full functional potential
    • Memory care for patients with Alzheimer disease and other types of dementia supported by staff who would be trained in dementia care that would also include sensible security measures for the safety of client
    • End-of-life or hospice services by providing medical, emotional, and spiritual support for patients nearing the end of life. These are supportive services covering documentation of wishes for treatments at the end of life and internment after passing.
